Heartland Community Health Center is a Federally Qualified Health Center that operates an integrated primary care practice that exists to serve all community members regardless of income or insurance status in Douglas County and the surrounding area. Heartland provides primary care services, behavioral health and dental as a NCQA recognized Patient Centered Medical Home. Additionally, Heartland offers the following health and wraparound services for all patients: Medication Assisted Treatment for Substance Use Disorder, Peer Support Services, Ryan White Medical Case Management, Compass – Insurance Eligibility Assistance, Nutrition Education Classes, Food Pantry, Spanish Language medical interpretation and Care Coordination. Heartland also offers discounted medication through its inhouse repository and 340B Pharmacy program. As the safety-net provider for the area, Heartland is the trusted provider of high quality, integrated care. Heartland has a patient-majority board of directors and strives to successfully meet the needs of the community.
