The Hospice RN Case Manager oversees the primary care plan for Hospice patients, delivering skilled professional nursing care to them, their families, and caregivers. This role includes facilitating patient and family education to ensure continuity of care and documenting the patient's condition and care provided accurately and timely.
Candidates must be graduates of an accredited registered nursing program, preferably holding a Bachelor's degree in Nursing, and possess two to four years of experience as an RN, ideally in hospice or home health. A valid driver's license and reliable, insured transportation are mandatory.

Job Requirements Graduate of an accredited registered nursing program Bachelors degree in Nursing preferred Minimum of 2 to 4 years as a RN with experience in a hospice or home health agency or other related health program. Experience with pain and symptom management interventions preferred Ability to work within an interdisciplinary setting Understanding of Hospice philosophy, principles of death/dying..
