The Physical Therapist will be responsible for evaluating residents, developing individualized treatment plans, and providing physical therapy services to both short-term rehab and long-term care patients. This role also requires accurate documentation and collaboration with interdisciplinary care teams to optimize patient outcomes.
Candidates must possess an active California Physical Therapist license and be a graduate of an accredited Physical Therapy program. Experience in a Skilled Nursing Facility or geriatrics is preferred, along with strong communication and documentation skills, and the ability to start immediately.
