The MDS Nurse (LPN) is responsible for coordinating the Resident Assessment Instrument (RAI) process, which includes completing an accurate Minimum Data Set (MDS) and developing the Interdisciplinary Plan of Care under the Director of Nursing's direction. This role also involves participating in daily Medicare PPS Interdisciplinary Team Meetings and maintaining positive relationships within the healthcare team.
Candidates must possess a current, unencumbered LPN license as required by the state and ideally have over one year of direct nursing and MDS/RAP experience. The position requires the ability to utilize local and consultant resources effectively and develop adequate knowledge of state, federal, and regulatory requirements.
