The role involves leading the clinical journey by implementing and evolving personalized care plans to ensure consistent, high-quality treatment tailored to resident needs. This includes acting as the clinical coordinator between physicians, specialists, and families while championing resident safety and managing discharge planning.
Candidates must possess a Registered Nurse license obtained prior to the hire date and a Diploma from an accredited school/college of nursing or equivalent professional licensure. A BLS Provider certification from the American Heart Association or American Red Cross is required within 30 days of hire.

How you’ll make an impact in this role Lead the clinical journey by implementing and evolving personalized care plans that ensure every resident receives consistent, high-quality treatment tailored to their unique needs. Act as the clinical "North Star" by coordinating between physicians, specialists, and families to ensure all disciplines are aligned and the patient’s voice is always heard. Champion resident safety by proactively identifying health risks or environmental hazards and taking immediate, decisive action to maintain a secure healing space. Advocate for successful transitions by assessing and managing discharge planning early, ensuring patients and families have the resources and confidence they need for the next stage of their journey.
BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Registered Nurse obtained prior to hire date or job transfer date required.
