Job Description

Your Impact: Deliver Field-Based Care: Conduct comprehensive primary care visits and physical exams in the comfort of a patient’s home. Manage Chronic Disease: Create longitudinal care plans that move the needle for high-risk patients. Collaborate Across the Continuum: Partner with an interdisciplinary team to ensure seamless, safe care transitions. Prevent ER Visits: Treat acute needs in the home to keep seniors safe, healthy, and out of the hospital. Job Requirements What You Bring: Advanced Degree: Master’s Degree (NP) with a preferred focus on Adult, Geriatric, and/or Family. Pennsylvania Credentials: Active, unencumbered NP license and Board Certification (ANCC or AANP). Reliable Mobility: Valid driver’s license and vehicle for local field-based travel. Requirements

Candidates must have a Master's Degree in Nursing with a focus on Nurse Practitioner and an active Nurse Practitioner license. Additional requirements include board certification, BLS certification, and a valid driver's license.
