Opportunity Information Sutter Medical Group of the Redwoods (SMGR) is recruiting a Board-Certified Geriatrician to join our established Geriatric practice within Sutter Senior Care. This is an exciting opportunity to practice in a team-based, value-driven outpatient setting focused on delivering high-quality care to older adults, particularly those with complex medical and social needs. The ideal candidate will have experience in a geriatric group practice or multi-specialty outpatient setting, with a strong background in primary care geriatrics and interdisciplinary team-based care. Position Highlights: Established Senior Care clinic model within a value-based care framework Focus on complex, vulnerable Medicare populations Collaborative multi-disciplinary care teams Opportunity to impact quality, outcomes, and patient experience Mission-driven environment supporting healthy aging and dignity in care Outpatient-only setting Monday through Friday clinic schedule Shared call with physician and APC team Emphasis on value-based, panel-oriented care Key Responsibilities: Provide comprehensive outpatient care for geriatric patients Diagnose and manage chronic and complex conditions (e.g., diabetes, hypertension, heart disease, cognitive disorders) Deliver preventive care, screenings, and immunizations tailored to seniors Manage polypharmacy and medication safety Collaborate with APCs, nurses, social workers, and specialists to deliver coordinated care Coordinate referrals and support care transitions across settings Educate patients and caregivers on disease management and healthy aging Participate in team huddles, quality initiatives, and value-based care programs

Qualifications

Requirements

Requires an MD or DO with Board Certification in Geriatrics, Internal Medicine, or Family Medicine and a valid California medical license. Experience in outpatient geriatrics and interdisciplinary team-based care models is preferred.
