The Medical Case Manager will develop close relationships with patients, coach them, and facilitate access to core and support services. They will also identify, track, and monitor clients lost to care while assisting them with medication adherence and navigating the healthcare system.
Candidates should have a high school diploma with relevant experience, and familiarity with people living with HIV is mandatory. A valid driver's license and the ability to access community resources are also required.
Basic knowledge of HIV/AIDS Be a team player and well-organized Skills in engaging people living with HIV Ability and willingness to use both knowledge and personal experience in facilitating linkage of individuals and groups to essential HIV health related services Communication skills and proven behaviors consistent with maintenance of health and wellness. Ability to establish trust based relationships with people in an appropriate and ethical manner Knowledge and ability to access community resources Ability to attend mandatory staff meetings Basic computer skills and ability to collect data elements and generate reports in a timely manner Motivated to serve people with HIV and get trained as assigned by Executive Director or supervisor Education and Experience: High school diploma with commensurate experience; associate or bachelor degree welcome Familiarity with people living with HIV is mandatory Familiarity with Nashville/Davidson and/or surrounding Counties Familiarity with Ryan White programs and Meharry Community Wellness Center is highly desirable Must possess valid driver’s license Willingness to attend all required trainings Reliability, punctuality, and discipline Environmental Conditions and Physical Demands Usual office environment.
