The Social Worker Care Coordinator is responsible for managing discharge coordination to ensure smooth transitions of care, supporting patient outcomes and hospital efficiency. This role involves evaluating patient clinical and psychosocial needs, identifying risks, and collaborating with physicians and nurses to tailor specific care plans.
Candidates must possess a Master's Degree in Social Work and hold active Social Work licenses in both Iowa and Illinois, with the second state license obtainable within 30 days of hire. A minimum of 1-2 years of post-graduate experience is required, along with CPR certification upon hire and a valid driver's license for work-related driving.
Take charge of discharge coordination to ensure smooth, timely transitions of care that support both excellent patient outcomes and efficient patient flow within the hospital. Evaluate each patient’s clinical condition and psychosocial needs, identifying risk factors and designing a plan of care. Pinpoint essential interventions and work closely with physicians and primary nurses to tailor care plans to the patient’s specific situation and goals. Build trust and collaboration by working directly with patients and their loved ones, ensuring their voices are heard and their needs are met. Play a central role in care coordination rounds, complex care discussions, and unit-based team meetings, promoting clear communication and unified planning across the care team.
Education: Master's Degree in Social Work required. License(s)/Certification(s): Social Work license in Iowa and Illinois required. License in second state must be obtained within 30 days of hire. CPR certiciation upon hire. Valid driver's license when driving any vehicle for work-related reasons.
