The Transplant Coordinator will coordinate care for post-transplant patients, facilitate clinic flow, manage transplant evaluations, and provide weekly educational sessions to patients and support members. This role involves managing a patient caseload, serving as the primary point of contact through the transplant process, recovery, and at home, including admitting and discharging patients.
Candidates must be a Registered Nurse with a BSN and possess at least two years of acute care experience, along with good medical and renal knowledge, and an understanding of chronic disease. A BLS certification is required, and the Certified Clinical Transplant Coordinator (CCTC) certification must be obtained within three years of hire.
