The Registered Nurse (RN) plays a vital role in providing comprehensive, patient-centered care within our multidisciplinary health team.
This position supports CFHC’s mission by delivering high-quality, evidence-based nursing care; coordinating health services across acute, chronic, and preventive care; and fostering long-term wellness through health education and patient advocacy.
Working collaboratively with medical providers, behavioral health, and care coordination staff, the RN ensures that each patient receives personalized support — from maternity care and family planning to chronic disease management and health promotion.
The RN will perform clinical assessments, dispense prescribed birth control per protocol, manage care transitions, and coach patients toward self-management goals.
This role also emphasizes proactive engagement with patients and families, integrating home visits, case conferencing, and data-driven quality improvement initiatives.
