The Nurse Practitioner or Physician Assistant will diagnose and treat acute, episodic, or chronic illnesses as part of a healthcare team within Senior Care Facilities. They will also analyze patient histories, recommend interventions, and educate patients about self-management of their health conditions.

Under the direction of the Clinical Operations, the NP diagnoses and treats acute, episodic, or chronic illnesses, as part of a healthcare team within Senior Care Facilities.
The provider will be responsible for servicing the Canton, Normal, Eureka, Peoria area.
Minimum Requirements: Current and valid NP/PA licensure in IL Access to insured, reliable transportation 6 months of prior NP/PA work experience Preferred Requirements: 2+ years Nursing Home Experience 2+ years of Cardiology or Pulmonary experience 2+ years of prior NP/PA work experience Professional Skills: Compassionate Resilience Integrity and Humility Inspired Innovation Demonstrate good judgment and problem-solving. Effective planning, communication, and organizational skills
