Job description

Key Duties and Responsibilities
Assists with the provision of palliative care in accordance with local, state, and federal regulations.
Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs.
Implements interventions to support the patient with chronic conditions to decrease recurring hospitalizations.
Collaborates with care team to manage referrals, follow up calls, and coordination of care.
Facilitates the patient’s transition within and between health care settings, e.g., admitting, transferring, and discharging patients.
Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.
Monitors the effectiveness of interventions.
Functions independently to perform age-appropriate history and physical for complex acute, critical, and chronically ill patients.
Initial home evaluation and monthly visits.
Attend IDT, and other team meetings as required.

Education and Experience
Will consider less NP experience with RN hospice and/or homecare experience.
Minimum of 5 years clinical hospice and palliative care experience. 2 years’ experience in the Nurse Practitioner role preferred.
Must work well independently and within a team.
Strong computer skills, including Word, Excel, and PowerPoint.
At least 2 years NP experience with hospice and/or palliative care experience (either NP or RN).
Current Advanced Practice Registered Nurse (APRN) Licensure.
Awareness about UM standards, NCQA requirements, CMS guidelines, Milliman guidelines, and Medicaid/Medicare contracts and benefit systems is helpful.
Nurse Practitioner certification in Hospice or Palliative Care within 1 year of starting the position.
Graduate of an accredited School of Nursing.
Knowledge of clinical standards of care.
Education and/or Experience: Completion of Nurse Practitioner accredited program.
Knowledge of long-term care, community resources and cost-effective alternatives regarding patient care delivery systems.
Minimum of two (2) years clinical experience, desired.
Exceptional interpersonal skills & superb communication a must.
At least one (1) year of experience in Hospice or Palliative Care.

Licenses/Certifications
Must be Board certified.
