The Physical Therapist Assistant will work under supervision to deliver physical therapy treatment interventions and services based on an established plan of care, focusing on functional and measurable outcomes. Responsibilities also include monitoring patient progress, reporting changes to the Physical Therapist, and maintaining accurate patient records.
Candidates must have successfully completed an accredited Physical Therapy Assistant program and possess a current, valid Physical Therapist Assistant license for the appropriate state. Strong communication skills, the ability to work in a team, and a compassionate, patient-centered approach are also required.

Blue Stone Therapy is seeking a Physical Therapist Assistant (PTA) to provide rehabilitation services to adult and geriatric patients at Chariton Specialty Care.
This role focuses on delivering skilled physical therapy services to adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Physical Therapy Assistant works under the supervision of a licensed Physical Therapist to implement treatment plans designed to improve mobility, strength, balance, endurance, pain management, and overall functional independence.
What You’ll Do Provide skilled physical therapy treatment in accordance with established plans of care Implement individualized treatment programs focused on functional and measurable outcomes Treat adult and geriatric patients in a skilled nursing facility (SNF) setting and rehabilitation settings Monitor patient response to treatment and communicate progress with the supervising Physical Therapist Maintain timely, accurate documentation to support quality care and reimbursement standards Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes
What You Bring Graduate of an accredited Physical Therapy Assistant program Current Physical Therapy Assistant license (or eligibility for licensure) in applicable state Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred Strong communication, organization, and clinical reasoning skills Commitment to patient-centered care and professional standards Why Blue Stone Therapy?
