• Position
• Highlights: Accurately assesses clients at admission and other required time points and utilizes appropriate standardized documentation. Assesses the client’s condition and needs and determines the agency’s ability to meet those needs in the home care setting. If therapy only case, informs the client/caregiver about the agency, care/services available, client obligation for payment, and rights and responsibilities before care is initiated. Makes appropriate referrals for evaluation/care to other disciplines and services, and coordinates care with others to ensure effective and efficient care is provided. Reports changes in client condition as appropriate and in a timely manner, to the client’s physician and obtains orders for changes in the plan of treatment. Initiates and implements appropriate preventive, therapeutic and rehabilitative procedures, and techniques to clients. Performs manual therapeutic exercises, massage, and/or manual traction as established by the plan of care.
