The Nurse Practitioner or Physician Assistant will act as a crucial member of an integrated health team, providing well-coordinated and high-quality care to PACE participants. Key duties include completing required documentation, participating in Interdisciplinary Team meetings, and managing administrative tasks like lab reports and medication refills.
Candidates must possess a Master's degree and a minimum of 2 years of experience working with underserved, low-income older adults in an outpatient setting. Necessary credentials include current unrestricted licensure in California, DEA license, and current American Heart Association BLS certification.
Complete all required documentation in accordance with department standards Work effectively with patients from diverse social, cultural and economic groups Participate in PACE Interdisciplinary Team (IDT) meetings Maintain all necessary certifications and licensures required Attend all mandatory meetings; minimizing incident reports; using resources efficiently and practicing medicine in a cost-effective manner. Maintain FHCSD behavioral standards, including (but not limited to), effective listening and communication skills and respectful interactions with colleagues and patients. Maintain satisfactory peer reviews: able to assess medical information including pathophysiology, patient presentation, differential diagnosis, etc., in order to facilitate health promotion and disease prevention. Meet clinic site and organization needs in a reliable and flexible manner; prioritizes work and makes an effort to adhere to schedule. Participate in Clinical Outcomes Program and work towards meeting organizational targets for all metrics. Participate in administrative duties, including lab reports, charts, and medication refills reviewed and managed appropriately. Perform other duties as assigned. Requirements Minimum of 2 years of experience working with underserved, low-income older adult persons in an outpatient clinic or similar setting required.
