The Care Coordinator will be responsible for coordinating care for long-term care residents using the company's EMR, maintaining proactive on-site engagement, and collaborating closely with providers within the facility setting. This role also involves serving as a key liaison between providers, staff, residents, and families, while accurately documenting all care coordination activities.
Candidates must be legally authorized to work in the US and possess an active clinical certification or licensure such as PCT, CNA, MA, LPN, or RN, along with a minimum of one year of patient-facing experience in a long-term care setting. The role requires full-time, on-site work in Reston, VA, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST, and necessitates a valid driver's license and reliable vehicle for potential travel between facilities.
Legally authorized to work in the United States of America Active, unrestricted clinical certification or licensure, such as: Patient Care Technician (PCT) Certified Nursing Assistant (CNA) Medical Assistant (MA) / Certified Medical Assistant (CMA) Licensed Practical Nurse (LPN) Registered Nurse (RN) Or comparable credential Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and in person Cultivate and maintain positive, approachable relationships with residents, families, providers, and facility staff, while demonstrating a consistent, dependable presence within the long-term care setting Well-developed organizational and time-management skills, paired with an optimistic approach and initiative-driven mindset Strong working knowledge of healthcare terminology and care coordination principles Proficiency with Microsoft 365, including Outlook, Word, and Excel Ability to work full-time, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST, in an on-site role, based in the Reston, Virginia area No weekends No on-call Daytime hours Seven (7) company-paid holidays annually Transportation criteria: Valid, unencumbered driver’s license Reliable personally-owned vehicle Current auto insurance, with the ability to provide proof Please note, travel between facilities may be required Ability to pass a background check and drug screen Full-time benefits eligibility with a variety of options, including a Wellness Pet Plan for your furry family members!
