The Registered Nurse provides comprehensive nursing care, assessing physical, social, and psychological needs to develop and coordinate cost-effective, multidisciplinary care plans for assigned patients. Responsibilities also include supervising paraprofessionals, managing documentation, and ensuring compliance with regulations while traveling to patient homes.

Deliver personalized nursing and care management to patients in their home or care facilities. Constantly evaluate evolving patient needs and respond with plan of care adjustments.
