This role supports care coordination activities within ambulatory services, primarily assisting with and facilitating the transition of care from inpatient settings back to home. Responsibilities include developing collaborative care plans, providing patient/family education, and implementing cost-effective problem-solving initiatives.
Minimum requirements include 3 years of experience in the current unit or specialty area, and a Bachelor of Science in Nursing (BSN) or equivalent experience. Candidates must possess strong analytical and data management skills, proficiency with electronic medical records, and excellent organizational and communication abilities.
This role supports care coordination activities within ambulatory services; specifically for the medical group. Performs all duties as assigned.
