The Nurse Practitioner will visit members within their assigned geographic area to perform in-home health risk assessments, which include medical history evaluation, systems review, medication reconciliation, vital signs checks, and a comprehensive physical exam. This practice involves point-of-care testing and identifying diagnoses for care management and active medical treatment.
Candidates must possess a valid, unencumbered Michigan Nurse Practitioner license and have a minimum of three years of patient care experience, with preference given to experience in primary care, adult, or geriatric settings, along with EMR skills. Essential soft skills include a strong sense of empathy, compassion, friendliness, and exceptional written and oral communication abilities.

You will visit members within your geographic area of responsibility and conduct an in-home health risk assessment including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and a comprehensive physical exam. This evidence-based practice screening will include point-of-care testing (as appropriate), the identification of diagnoses to be used in care management by our Porter Care Guides and active medical management in the furtherance of treatment, and the formulation of a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment.
