About the role

The Staff Physical Therapist is responsible for conducting initial patient evaluations, reassessments, and discharge summaries, and administering treatment based on these findings. They must also complete required documentation promptly and maintain departmental productivity standards.

Upper Valley Medical Center Outpatient Physical Therapist PRN - Days | Hours vary between 8 am - 6:30 pm Under the general supervision of the department Supervisor in Rehabilitation Services and according to established policies and procedures, the Staff Physical Therapist will: conduct patient initial evaluation, reassessments, and discharge summary for all populations served; administer patient treatment based upon evaluation and/or reassessment; complete required documentation in a timely fashion; maintain productivity at departmental standard; delegate tasks appropriately to personnel; participate in the case management of patients’/clients’ rehabilitation services; perform non-clinical tasks and demonstrate work habits to support effective daily operations; participate in one to two departmental, hospital, and/or community programs/meetings; and teach, orient, train, supervise and mentor co-workers, new employees, volunteers, observers, and a minimum of one student annually.

Education

Must be a graduate of an accredited Physical Therapy Program.

Licensure

Possess a valid license to practice Physical Therapy issued by the State of Ohio.
