The Nurse Practitioner delivers comprehensive patient care in a primary care setting, following evidence-based guidelines to diagnose, treat, and monitor both acute and chronic conditions. They collaborate with other providers and ensure continuity of care through treatment plans and patient education.
Candidates must have a Family Nurse Practitioner master's degree or Doctor of Nursing Practice with an active Arizona license, along with at least 1 year of recent Primary Care or Family Practice experience. Experience in a Federally Qualified Health Center or Primary Health Care environment is preferred.
Leadership Team Serves as a member of the medical staff team, communicating on a regular basis with the Medical Director, reporting on medical issues, and serving as a consultant to the senior managers. Participates in the development of the strategic plan, identifying requirements for new programs, equipment, facilities and/or replacement of major items. Assists in maintaining fiscal stability of the corporation by participating in the planning and budgeting process with the management staff to identify the opportunities, priorities, goals, and objectives of health care provided by the Health Center. Works with the leadership team in the development of a high quality clinical information system that includes thoroughly documented medical records, tracking of referrals, recall of patients, and maintenance of Quality Improvement Programs and assessment of clinical outcomes. Participates in implementing the health care plan. Should participate in monthly providers meeting and other meetings as assigned by Medical Director. Clinical Care Provides supervision and leadership for the medical support staff and clinical operations. Provides direct patient care and supervises FNP’s in accordance with the specific medical specialty job description. Maintains current licensure and board certifications. Maintains after-hours availability for staff consultation on patient care issues as relates to policies and procedures. Contributes to team effort by fulfilling on-call duties and hospital hours, if applicable. Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks and participating in professional societies. Performs patient care responsibilities in accordance with the highest and generally accepted professionalism shown. Effectively communicate with other providers to ensure continuity of care. Medical Policy Develops, implements, and evaluates adherence to written clinic policies, protocols and procedures concerning clinic operations, staffing patterns and coverage, case management systems, and risk management procedures (see quality improvement below). Adopts and helps in implementing practice guidelines (see quality improvement). Quality Improvement Helps develop, implement and review adherence to a quality improvement program that assesses the following items: continuous quality improvement (clinical and service indicators), utilization management, standard practice guidelines, peer review, incident reporting, provider satisfaction, patient satisfaction, patient grievances, policy and procedures, nurse practitioner / physician assistant protocols, credentialing, internal audits, and patient education. Assists in conflict resolution and the daily flow of the clinic as needed. Promotes education of medical and non-medical staff in relevant topics. Promotes the acceptance and placement of students from health related disciplines, if applicable. Works closely with supervisors responsible for management and performance of support staff. The Medical Provider shall perform other duties as assigned by the Medical Director or Chief Executive Officer.
