The Nurse Practitioner or Physician Assistant will deliver high-quality primary care in skilled nursing and senior living facilities by performing comprehensive assessments, managing chronic conditions, and conducting routine rounds. Responsibilities also include collaborating with staff and families for coordinated care and maintaining accurate documentation in the EMR system.
Candidates must possess a valid Nurse Practitioner or Physician Assistant license and current board certification in their field, along with a Master's Degree in Nursing or Physician Assistant Studies. Strong skills in leadership, communication, problem-solving, and organization are preferred, and the role requires travel to assigned facilities.
Psychiatric Care and Rounding: Conduct patient rounds and manage psychiatric care within SNF, LTC, AL, and IL settings. Collaborative Care Coordination: Partner with attending physicians, consulting specialists, referral sources, and facility staff to ensure comprehensive, coordinated care. Patient Evaluation and Management: Assess and manage the mental health needs of residents, perform physical exams, and gather complete medical histories. Diagnostics and Therapeutics: Order and interpret lab and diagnostic tests, make preliminary diagnoses, and develop effective therapeutic care plans for mental and physical health. Medication and Treatment Management: Prescribe and administer medications and treatments; provide injections as necessary. Patient and Family Education: Educate patients and families on medications, care plans, and preventive mental health practices. Acute Care Collaboration: Work alongside physicians and facility teams to address acute psychiatric and medical needs promptly and effectively.

Graduation from an accredited Nurse Practitioner program and ANCC certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP). Active DEA license. BLS and ACLS certification. Current Nurse Practitioner license in the state of practice. This role requires travel to and from one or more assigned facilities. Depending on the amount of travel required, Curana may in future determine that a valid driver’s license, current auto insurance, and an acceptable driving record are required.
