The Social Worker is responsible for assessing, counseling, intervening, implementing, and educating patients and their families to facilitate movement throughout the continuum of care. This role emphasizes a patient-focused approach to care delivery.

Candidates must have a Bachelor’s degree and 2-4 years of related experience. A NJ Licensed Clinical Social Worker certification and knowledge of discharge planning and Medicare/Medicaid regulations are required.
