The Nurse Practitioner will primarily conduct in-home, patient-facing visits, performing primary care services, initial screenings, examinations, creating assessment plans, and completing timely documentation. They will partner with other clinicians to manage patient health, engage attributed patients in the WellBe program, and address urgent care needs as required.
Candidates must possess a Master's Degree in Nursing with a focus on Family, Adult, Geriatric, or Adult-Geriatric Nurse Practitioner from an accredited program, along with an active, unencumbered Nurse Practitioner license in Wisconsin and current board certification. Required credentials include BLS, CPR certification, an active NPI#, and an active, unencumbered DEA licensure or eligibility.
Your Impact: Deliver Field-Based Care: Conduct comprehensive primary care visits and physical exams in the comfort of a patient’s home. Manage Chronic Disease: Create longitudinal care plans that move the needle for high-risk patients. Collaborate Across the Continuum: Partner with an interdisciplinary team to ensure seamless, safe care transitions. Prevent ER Visits: Treat acute needs in the home to keep seniors safe, healthy, and out of the hospital. Job Requirements What You Bring: Advanced Degree: Master’s Degree (NP) with a preferred focus on Adult, Geriatric, and/or Family. Wisconsin Credentials: Active, unencumbered NP license and Board Certification (ANCC or AANP). Reliable Mobility: Valid driver’s license and vehicle for local field-based travel.
