The physician will provide outpatient care to patients aged 18 and up, focusing on continuity of care and strong patient-provider relationships. They will also be part of a medical home model supported by a network of specialists and services.
Candidates must be board certified or board eligible in Family Medicine or Internal Medicine, with experience preferred. The position is designed for a physician who values a supportive and rewarding work environment.
