The role involves providing quality living and community support by teaching, supporting, and facilitating choices for individuals in residential or community settings, primarily by implementing Person Centered Support Plans. Major duties include assisting with daily living skills such as meal preparation, hygiene, housekeeping, and administering medication and treatments.
Prior direct care experience is preferred but not required, and a High School diploma or equivalent is preferred. Candidates must be able to lift, push, or pull a minimum of 50 pounds, stand for periods, bend, push wheelchairs, and provide physical interventions regularly.
Implement Person Centered Support Plans for assigned individuals. Assist with daily living skills including but not limited to, meal preparation, general hygiene, toileting, general housekeeping, work and leisure activities. Administer medication and treatment(s). Complete documentation as required through the general course of duties. Report to the appropriate Level 2 of Level 3, Program Manager, Case manager or Wellness Liaison any occurrences that affect the immediate well-being of the individual. Attend all meetings as assigned and required. If approved as a driver of agency vehicles, employee may transport individuals. Other duties as assigned.
