Provide clinical social work and case management services to oncology patients to address psychosocial barriers to care. Conduct age-specific assessments and develop intervention plans to support patients through diagnosis and disease progression.
Under the general supervision and direction of the Director, Oncology Nursing or designee, provide clinical social work services to patients and families assisting to remove barriers to care by providing interventions/referrals for financial and transportation issues. Provide case management services independently including but not limited to application of clinical diagnostic, assessment or treatment knowledge or skills. Identify and address psychosocial factors that may impede the patient�s ability to optimize benefit from the health care delivery experience. Provide age specific assessments of psychological and social functioning including determining various psychiatric diagnoses according to current DSM criteria and establishing intervention plans to address the identified issue. Screen and evaluate patients and their families for psychological distress during diagnosis and disease progression.
A Masters Degree in Social Work from an accredited college or university required. Current Massachusetts Social Work license at a minimum of a LCSW is required. LICSW preferred. Proficient skills in Microsoft Office Word are required. Three (3) years medical/psych/SA experience required, preferably in a hospital setting. LCSW minimum, LICSW preferred. Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.
Pay rate will be determined based on level of experience. Pay Range USD $33.66 - USD $46.52 /Hr.
