• What you will do: Promote/provide Occupational Therapy services in the home to assist the patient in achieving his/her optimal level of functioning. Evaluate and treat individuals with functional deficits secondary to neurologic, orthopedic and medical conditions. Teach patients/caregivers and help them to become independent with their treatments and self-management of their chronic illness(es) Coordinate an interdisciplinary plan of care with nursing/physical/occupational/speech therapists, medical social workers, aides and dieticians. Working hours are primarily M-F generally 8am to 4pm, some flexibility in schedule
• Minimal weekend and holiday rotations
• What you will need: NYS Occupational Therapist Licensure 1 year Occupational Therapy experience is required Home care experience preferred but not required Valid Driver's License and Reliable Vehicle. This position provides mileage reimbursement. Care for one patient at a time.
