The Occupational Therapist is responsible for developing personalized treatment plans and ensuring that all treatment is delivered according to the established plan of care. They will monitor patient responses and modify treatment approaches as necessary while ensuring thorough documentation.
Candidates must hold an active Illinois license in Occupational Therapy and preferably have experience in skilled nursing facilities or long-term care. Current knowledge of treatment practices and familiarity with the Net Health Electronic Documentation System is preferred.

Developing personalized treatment plans using a variety of treatment techniques to maximize safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL). Ensure all treatment delivered is in accordance with the established plan of care. Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration. Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
Occupational Therapist Qualifications: Active/Valid Illinois license in Occupational Therapy SNF/Long term care experience preferred. Current knowledge of treatment practices Knowledge of Net Health Electronic Documentation System preferred.
