The Licensed Practical Nurse delivers care in a community setting by utilizing the nursing process, which includes assessment, planning, intervention, implementation, and evaluation. Responsibilities also involve training employees on medical needs, monitoring health conditions, and ensuring accurate documentation and medication administration.
Candidates must possess a valid nursing license and driver's license with a satisfactory driving record, along with a minimum of one year of experience providing nursing services to individuals with disabilities. A Bachelor's degree or equivalent experience in nursing is required, and employment is contingent upon successful completion of various background checks and drug testing.
Provide treatment and health care procedures ordered by a physician and required by the standards of professional practice or law Ensure individuals attend medical appointments per their Individual Support Plan (ISP) Advise supervisor of all concerns and issues related to the provision of nursing services Develop and update Individual Support Plans (ISP)/Person Centered Plans (PCP) with an interdisciplinary team to include any concerns, observations, and behavioral developments Train employees on the medical needs of assigned individuals Monitor health and medical conditions of individuals and refer for medical services as needed Monitor the use of medications through medication administration logs Ensure program documentation and billable records are completed accurately and timely Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families Additional tasks and responsibilities may be assigned Employee must cooperate with the license and department staff in any inspection, inquiry, or investigation Minimum Requirements Bachelor’s degree or equivalent experience in nursing field Minimum of one year of experience providing nursing services to individuals with disabilities Valid nursing license Valid driver’s license with a satisfactory driving record Ability to calculate figures related to medication dosages Ability to establish a comfortable and supportive relationship with an individual receiving supports Ability to effectively present facts and recommendations Employment is contingent upon successful completion of checks of criminal background, central registry, child abuse registry, and drug testing Why Community Options?
