Clinical
Responsibilities: Perform comprehensive assessments, establish diagnoses, and develop or adjust treatment plans tailored to patient age, acuity, and clinical condition. Order, interpret, and act on diagnostic studies; prescribe pharmacologic and non-pharmacologic interventions in alignment with best practices. Manage complex conditions using evidence-based guidelines, balancing clinical indications, cost-effectiveness, and risk-benefit considerations. Provide health promotion, disease prevention, and patient/family education to support long-term management of heart failure. Support initiatives aimed at reducing cardiac-related hospital admissions through proactive management and patient education. Collaborate within an interdisciplinary team, seeking physician input or specialty referrals as appropriate; escalate care for emergent needs. Maintain accurate, timely documentation in the electronic medical record and ensure compliance with coding and billing standards. Facilitate coordinated care and clear communication among all care team members and community resources. Perform office or hospital-based procedures within granted privileges and demonstrated competencies.
