About the role

The Licensed Vocational Nurse performs nursing care as directed by the Registered Nurse Case Manager to ensure the comfort and health of patients and families in a home health and hospice setting. Responsibilities include coordinating patient care, documenting patient-related information, and collaborating with an interdisciplinary team.

Summary

Performs the nursing care plan as directed by the Registered Nurse Case Manager to maximize the comfort and health of Home Health/Hospice patients and families.

Essential Functions

⦁ Assists the RN Case Manager (RNCM) to ensure quality and safe delivery of home health /hospice nursing services.

⦁ Performs the nursing care plan as directed by the Registered Nurse Case Manager including nursing services, treatments and preventative and rehabilitative procedures; performs procedures at a level of care standard within the medical community, scope of practice consistent with the Vocational Nursing Practice Act.

⦁ Obtains data on physical, psychological, social and spiritual factors that may influence patient health status and incorporates that data into the plan of care; evaluates progress towards desired outcomes and updates the patient’s plan of care as appropriate to accommodate changing patient and family needs.

⦁ Coordinates physical care of the patient by teaching and/or training patients, primary caregivers, volunteers, employed caregivers, and by providing direct care as appropriate.

⦁ Communicates with RNCM on a regular basis regarding changes in the patient’s condition and/or needs; informs the RNCM of unusual or potentially problematic patient and family issues.

⦁ Completes patient-related documentation within 24 hours of the shift worked; maintains up-to-date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur.

⦁ Collaborates and consults with the interdisciplinary group on issues related to patient care.

⦁ Participates in in-service training programs for professional staff, as well as quality assurance and performance improvement activities.

⦁ Ability to assess end of life signs/symptoms such as pain, anxiety/agitation, nausea/vomiting, trouble breathing, constipation/diarrhea, end of life & imminent death.

⦁ Ensures SHE and SHHCH policies are followed in the performance of duties.

Qualifications

⦁ Current California Licensed Vocational Nurse.

⦁ Minimum one year professional nursing experience in home health, hospice, acute or sub-acute preferred.

⦁ Able to work both independently and as part of an interdisciplinary team.

⦁ Good communication and interpersonal skills.

⦁ Bilingual in English and Cantonese preferred. Mandarin & Toishanese a plus.

⦁ Must pass health screening for TB clearance, physical examination, and background check.

⦁ Current CPR certification.

⦁ Must be able to travel via public transportation or personal vehicle for performance of job duties. If driving, must have and maintain a valid CA driver license, satisfactory driving record, and auto insurance coverage as specified in Self-Help's policy.

Requirements

Candidates must have a current California Licensed Vocational Nurse license and a minimum of one year of professional nursing experience in relevant fields. Good communication skills and the ability to work independently or as part of a team are essential.
