About the role

The Hospice Physical Therapist will plan, organize, and deliver therapy services focused on maintaining patient comfort, function, and quality of life, guiding patients and caregivers through supportive techniques and education at the end of life. Essential functions include assessing functional status, directing therapy treatments, instructing caregivers, and ensuring patient safety and comfort within the home environment.

What You’ll Do

  • Deliver Patient-Centered Care
  • Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential
  • Develop and implement individualized care plans in collaboration with physicians and the care team
  • Provide hands-on therapy to improve mobility, strength, balance, and independence
  • Educate patients and caregivers on treatment plans, exercises, and safety strategies
  • Drive Outcomes & Recovery
  • Monitor patient progress and adjust treatment plans as needed
  • Support safe transitions of care and contribute to discharge planning
  • Identify additional patient needs and collaborate with interdisciplinary teams
  • Collaborate & Communicate
  • Partner closely with nurses, therapists, and physicians to ensure coordinated care
  • Document patient care and progress accurately and timely within the EMR
  • Participate in case conferences and team meetings
  • Lead & Support
  • Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
  • Provide guidance and education to caregivers and support staff
  • What You Bring
  • Graduate of an accredited Physical Therapy program (DPT or Master’s)
  • Current PT license in the state of practice
  • Minimum 1–2 years of experience (home health experience preferred)
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and patient education abilities
  • Current CPR certification
  • Valid driver’s license, reliable transportation, and auto insurance

Requirements

Candidates must possess a Bachelor's or Master's degree in physical therapy from an accredited program and be licensed to practice as a physical therapist in the state of practice. Preferred qualifications include two years of relevant experience, particularly in home health or community-based settings, along with strong communication skills and a current CPR certification.
