The Physical Therapist initiates skilled physical therapy treatments based on physician orders, maintains necessary documentation including evaluations and progress notes, and confers with other healthcare staff regarding patient needs. This role also involves attending meetings, assisting with training, and assuming the duties of the Director of Physical Therapy when absent, including supervision.
Candidates must have graduated from an approved school of physical therapy and possess a valid State license to practice physical therapy, with two years of experience being preferred. The role requires the ability to direct others and the capacity to read, write, speak, and understand English at an intermediate or advanced level for effective communication.
Physical Therapist: Initiates skilled physical therapy treatments in accordance with physician’s orders; maintains required contact with physician. Maintains written evaluations, progress notes and discharge summaries for individuals in physical therapy program. Confers with other health care staff regarding special needs of individuals to enhance rehabilitation or maintenance of function. Provides input for meetings and attends during absences of the Director Physical Therapy. Maintains statistical records of individuals’ treatments. Assists with in-service training programs upon request. Records and updates individual’s plan of care in the Inter-disciplinary Kardex. Submits requisition for supplies needed in the treatment to Director Physical Therapy. Assumes all duties of the Director Physical Therapy during absences including supervision of Physical Therapy Aides and responsibility for all necessary documentation. From Up to
Physical Therapist: Graduation from an approved school of physical therapy required. State license to practice physical therapy required. Two years’ experience in physical therapy preferred. Ability to direct others necessary. This position requires regular interaction with residents, coworkers, visitors, and/or supervisors.
