The Home Health Nurse is responsible for planning, organizing, and directing home care and hospice services, which includes completing initial and regular patient assessments and developing/revising the plan of care. Key duties involve administering treatments, counseling patients and families, coordinating care as a Case Manager when assigned, and communicating updates to physicians and community health personnel.
Candidates must be graduates of an accredited school of nursing and hold current state licensure along with CPR certification, though one to two years of acute care experience is preferred. Essential requirements include strong communication and problem-solving skills, physical ability to perform nursing tasks like lifting and prolonged standing, and possession of a licensed, insured automobile.
The Home Health Nurse plans, organizes and directs home care and hospice services and is experienced in nursing, with emphasis on community health education/experience. The Home Health Nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
This position has no direct supervisory responsibilities.
