The Social Worker will conduct psychosocial evaluations of patients and families, implement treatment plans according to insurance guidelines, and provide end-of-life bereavement supportive services. Responsibilities also include assessing bereaved clients, discussing community resources, and participating in grief education.
Candidates must possess an MSW or BSW with appropriate licensure or certification, along with a minimum of one year of experience in a healthcare setting, preferably in Hospice or Home Health Care. A valid driver's license and insured vehicle are mandatory, along with demonstrated clinical skills in assessment and counseling.
Position Title: Full-Time licensed social worker
Employment is contingent upon successful completion of a background check, drug screening, and possession of a valid driver's license.
