This role focuses on providing in-home post-discharge care management and medical treatment for patients transitioning from acute care facilities back to their homes.
Key Responsibilities Conduct in-home patient visits to assess clinical status and provide post-discharge medical care. Review and interpret discharge summaries and progress notes.
