The Occupational Therapist will be responsible for reviewing medical records, evaluating patient abilities and needs, and developing and implementing individualized treatment plans to help patients regain independence. Key duties include providing ADL training, adaptive equipment consultation, environmental modifications, documenting progress per Medicare guidelines, and collaborating with interdisciplinary teams.
Candidates must possess a Bachelor's degree in Occupational Therapy and hold a current state OT license. Experience in Long-Term Care and strong knowledge of Medicare documentation are preferred qualifications.
Occupational Therapist (OT) - Long-Term Care LP Therapy LP Therapy is seeking a licensed Occupational Therapist (OT) to provide evaluation and treatment services in a Long-Term Care setting, helping patients regain independence and improve daily functioning.

Bachelor's degree in Occupational Therapy; current state OT license; LTC experience preferred; strong knowledge of Medicare documentation; excellent communication and organizational skills. Work Environment: LTC facility; physical demands consistent with therapy services.
