The Physical Therapist will be responsible for reviewing physician referrals, evaluating patient function, and developing and modifying individualized treatment plans. Key duties also include administering therapeutic modalities, educating patients and families, and completing necessary Medicare documentation.
Candidates must possess a Bachelor's degree in Physical Therapy and hold a current state PT license. Preferred qualifications include prior experience in Long-Term Care settings and a strong understanding of Medicare guidelines.
Physical Therapist (PT) - Long-Term Care LP Therapy LP Therapy is seeking a licensed Physical Therapist (PT) to provide skilled therapy services in a Long-Term Care setting focused on restoring mobility, strength, and function.
review physician referrals and medical records; evaluate strength, mobility, and functional ability; develop and modify treatment plans; administer therapeutic exercises and modalities; educate patients and families; complete timely Medicare documentation; collaborate with care teams; supervise PTAs and aides as required; support quality outcomes and productivity standards.
Bachelor's degree in Physical Therapy; current state PT license; LTC experience preferred; strong knowledge of Medicare guidelines; effective communication and teamwork skills. Work Environment: LTC facility; physical demands include lifting and mobility assistance.
