The Occupational Therapist will be responsible for reviewing medical records, evaluating patient abilities and needs, and developing/implementing individualized treatment plans to help patients regain independence. Key duties include providing ADL training, adaptive equipment consultation, environmental modifications, documenting progress per Medicare guidelines, and collaborating with interdisciplinary teams.
Candidates must possess a Bachelor's degree in Occupational Therapy and hold a current state OT license. Experience in Long-Term Care and strong knowledge of Medicare documentation are preferred qualifications.
Occupational Therapist (OT) - Long-Term Care LP Therapy LP Therapy is seeking a licensed Occupational Therapist (OT) to provide evaluation and treatment services in a Long-Term Care setting, helping patients regain independence and improve daily functioning.
review medical records; evaluate patient abilities and needs; develop and implement individualized treatment plans; provide ADL training, adaptive equipment, and environmental modifications; document progress per Medicare guidelines; collaborate with interdisciplinary teams; supervise COTAs as assigned; support patient satisfaction and quality outcomes.
Bachelor's degree in Occupational Therapy; current state OT license; LTC experience preferred; strong knowledge of Medicare documentation; excellent communication and organizational skills. Work Environment: LTC facility; physical demands consistent with therapy services.
