Role Overview

Are you a Physical Therapist who thrives on one-on-one patient care and the freedom of the open road? We are seeking a compassionate and autonomous Physical Therapist to join our home health team. In this role, you won't just be treating symptoms; you'll be helping patients regain their independence in the place they feel most comfortable—their own home.

What You’ll Do

Restore Independence

Conduct comprehensive initial evaluations and develop personalized plans of care focused on functional mobility and safety.

Clinical

Excellence: Provide high-quality manual therapy, therapeutic exercise, and gait training tailored to the home environment.

Collaborative

Care: Coordinate with physicians, occupational therapists, and nursing staff to ensure a holistic approach to patient recovery.

Documentation

Maintain timely and accurate clinical records (OASIS experience is a major plus!) via our electronic medical record system.

Education

Empower patients and their families by teaching home exercise programs and fall prevention strategies. What We’re Looking For

Credentials

Valid State License in Physical Therapy (PT).

Experience

Home Health experience preferred, but we are willing to train the right "hospital-to-home" transitioner.

Independence

A self-starter who can manage their own schedule and travel to various patient homes.

Communication

Strong interpersonal skills to build rapport with patients and families.

Reliability
