The Physical Therapist will conduct comprehensive initial evaluations and develop personalized care plans focused on functional mobility and safety for patients in their homes. Responsibilities also include providing high-quality manual therapy, therapeutic exercise, gait training, and coordinating care with other medical staff.
Candidates must possess a valid State License in Physical Therapy and demonstrate strong independence to manage their schedule and travel to various patient homes. Home Health experience is preferred, but training is available for suitable candidates, along with the requirement of a valid driver's license and reliable transportation.

Are you a Physical Therapist who thrives on one-on-one patient care and the freedom of the open road? We are seeking a compassionate and autonomous Physical Therapist to join our home health team. In this role, you won't just be treating symptoms; you'll be helping patients regain their independence in the place they feel most comfortable—their own home.
Maintain timely and accurate clinical records (OASIS experience is a major plus!) via our electronic medical record system.
Empower patients and their families by teaching home exercise programs and fall prevention strategies. What We’re Looking For
Valid State License in Physical Therapy (PT).
Home Health experience preferred, but we are willing to train the right "hospital-to-home" transitioner.
A self-starter who can manage their own schedule and travel to various patient homes.
Strong interpersonal skills to build rapport with patients and families.
