Performs physical exams at initial and follow-up appointments to assess patient needs and monitor progress. Prescribes medications and treatments to address diagnosed health conditions. Interprets clinical data to determine appropriate management and treatment strategies. Orders and evaluates diagnostic/laboratory tests, ensuring appropriate follow-up when abnormal results are identified. Facilitates ease of access to prevent unnecessary emergency room visits. Facilitates referrals to higher levels of care for complex medical concerns beyond the outpatient scope. Provides health promotion and disease prevention. Conducts routine check-ups and screenings. Provides/recommends/refers for routine immunizations/vaccinations. Develops and implements individualized patient management plans, ensuring continuity of care through detailed progress documentation. Prescribes and monitors medications for physical conditions, evaluating efficacy and managing potential side effects. Provides comprehensive education on diagnosis, treatment options (including lifestyle changes i.e. diet, exercise, smoking cessation), medication risks, benefits, and alternatives to support informed decision-making. Collaborates with medical staff and clinical teams to uphold quality standards and ensure effective care coordination. Coordinates with behavioral health providers and external healthcare teams to optimize patient care and treatment outcomes. Participates in peer review processes to ensure clinical excellence and continuous improvement in patient care.

• Knowledge: Possesses the equivalent of advanced formal training in nursing. Attained a Master’s Degree from an accredited nursing program. Must know all aspects of the trade as it is performed in the organization, sufficient to effectively train or instruct others, and to serve as a resource to employees.

• Education: Maintains current knowledge of standards of care and practices, typically acquired through continuing education.
• Licenses &
• Certifications: Certification as a Family Nurse Practitioner (FNP-BC), a valid Advanced Practice Registered Nursing (APRN) License in the state of West Virginia, and a valid state-issued driver’s license are required.
• Technical
• Skills: Prepares basic correspondence and simple reports in Microsoft Word. Uses Microsoft Excel to create tables and simple displays of information. Creates basic presentations in Microsoft PowerPoint. Demonstrates necessary proficiency with all electronic clinical systems, including EHR and scheduling systems, in use at the Company.
