Performs physical exams at initial and follow-up appointments to assess patient needs and monitor progress. Prescribes medications and treatments to address diagnosed health conditions. Interprets clinical data to determine appropriate management and treatment strategies. Orders and evaluates diagnostic/laboratory tests, ensuring appropriate follow-up when abnormal results are identified. Facilitates ease of access to prevent unnecessary emergency room visits. Facilitates referrals to higher levels of care for complex medical concerns beyond the outpatient scope. Provides health promotion and disease prevention. Conducts routine check-ups and screenings. Provides/recommends/refers for routine immunizations/vaccinations. Develops and implements individualized patient management plans, ensuring continuity of care through detailed progress documentation. Prescribes and monitors medications for physical conditions, evaluating efficacy and managing potential side effects. Provides comprehensive education on diagnosis, treatment options (including lifestyle changes i.e. diet, exercise, smoking cessation), medication risks, benefits, and alternatives to support informed decision-making. Collaborates with medical staff and clinical teams to uphold quality standards and ensure effective care coordination. Coordinates with behavioral health providers and external healthcare teams to optimize patient care and treatment outcomes. Participates in peer review processes to ensure clinical excellence and continuous improvement in patient care.

  • Knowledge: Possesses the equivalent of advanced formal training in nursing. Attained a Master’s Degree from an accredited nursing program.
