The Service Coordinator is responsible for coordinating programs and services to help clients maintain the highest practicable level of independence and well-being by assisting them in acquiring and utilizing necessary community services. This role involves acting as a liaison between agencies and providers, monitoring service delivery, maintaining detailed resident files, and conducting required quality service reviews and reporting.
A Bachelor's degree in social work or an Associate's degree in Nursing, Social Work, or Gerontology is preferred, along with two or more years of experience in social service delivery to the elderly population and working knowledge of local community resources. A valid driver's license is required, and proficiency in operating a computer and relevant software is necessary.
