The Physical Therapist is responsible for assessing patients, developing care plans, and providing physical therapy services to home care and hospice patients. They work as part of an interdisciplinary team and participate in discharge planning and patient education.
Candidates must have a degree in physical therapy, be licensed to practice, and have at least two years of experience, preferably in community or home health settings. They should also possess current CPR certification and strong communication and physical therapy skills.
The Home Health and Hospice Physical Therapist is responsible for assessing, developing a plan of care and providing physical therapy services to home care and hospice patients of all ages. The physical therapist is an essential part of an interdisciplinary team.
This position has no supervisory responsibilities.
