The Physical Therapist will be responsible for reviewing physician referrals, evaluating patient mobility and function, and developing and administering tailored treatment plans using therapeutic exercises and modalities. They must also educate patients and families while ensuring timely Medicare documentation is completed.
Candidates must possess a Bachelor's degree in Physical Therapy and hold a current state PT license. Preferred qualifications include experience in Long-Term Care settings and strong knowledge of Medicare guidelines, alongside effective communication and teamwork abilities.

LP Therapy is seeking a licensed Physical Therapist (PT) to provide skilled therapy services in a Long-Term Care setting focused on restoring mobility, strength, and function.
review physician referrals and medical records; evaluate strength, mobility, and functional ability; develop and modify treatment plans; administer therapeutic exercises and modalities; educate patients and families; complete timely Medicare documentation; collaborate with care teams; supervise PTAs and aides as required; support quality outcomes and productivity standards.
Bachelor's degree in Physical Therapy; current state PT license; LTC experience preferred; strong knowledge of Medicare guidelines; effective communication and teamwork skills.
LTC facility; physical demands include lifting and mobility assistance.
