The Social Worker is responsible for planning, organizing, and implementing social services for PACE participants and their families. This includes assessment, treatment, counseling, and advocacy to meet the needs of participants and caregivers.
Candidates must have a Master's degree in Social Work and at least two years of experience, including one year with the elderly population. A current social work license in the state is preferred, along with knowledge of social service principles and financing mechanisms.
Must possess a comprehensive knowledge of social work as normally obtained through a Master's degree in Social Work. Two (2) years of experience, including one (1) year working with the frail or elderly population. Current license in social work within the state of PACE Organization preferred but not required Experience working on a multidisciplinary team in a hospital, nursing home or community-based setting is preferred. Experience and thorough knowledge of social service principles and practices. Knowledge of financing mechanisms such as Medicare, Medicaid and other payment systems. Valid driver's license and ability to provide a reliable means of transportation required. Position may require 10% travel within the Centers geographic region. Position requires ability to engage in frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting. Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.
