Provide psychosocial support and assessments for pediatric oncology patients and their families to help them cope with illness and life-limiting conditions. Coordinate care transitions, manage referrals to community resources, and collaborate with healthcare teams to optimize patient outcomes.
A Bachelor's Degree in Social Work is required, with experience in a healthcare setting preferred. Candidates must possess strong critical thinking and creative problem-solving skills to navigate the continuum of care.
