The Targeted Case Manager assists eligible individuals in accessing necessary recovery supports, medical, social, educational, and other services by assessing needs and coordinating service provision. Key duties involve monitoring client well-being, connecting clients with resources, motivating goal achievement, and developing long-term care plans.
Candidates must possess a Bachelor's degree in social work or a human services field, along with one year of post-graduation work experience or a Master's Degree. Essential requirements include a current driver's license, basic computer skills, excellent documentation abilities, and the capacity to handle crisis situations calmly.
